WAIVER AND RELEASE OF LIABILITY
By signing this agreement, I acknowledge that I am voluntarily participating in a health and wellness event organized by The Reset Sanctuary, which may include physical activities such as Pilates, stretching, light aerobic movement, and other related wellness sessions.
I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participating in physical activities involves inherent risks, including but not limited to muscle strains, sprains, dizziness, fainting, or other potential injuries. I voluntarily assume full responsibility for any risks, injuries, or damages that I may sustain as a result of participating in this event.
2. Medical Clearance
I affirm that I am in good health and have consulted with a physician or other qualified healthcare provider, if necessary, regarding my ability to participate in physical activities. I will inform instructors of any medical conditions or limitations that may affect my participation.
3. Waiver and Release
I, for myself, my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless The Reset Snactuary, its officers, instructors, volunteers, sponsors, and other participants from any and all liability, claims, demands, causes of action, or expenses (including attorney's fees) resulting from any injury, illness, or loss I may sustain while participating in this event.
4. Photography & Media Release
I understand that photos or videos may be taken during the event for promotional purposes. By signing this waiver, I give permission for images of myself to be used in promotional materials, social media, and websites, unless I indicate otherwise in writing prior to the event.
5. COVID-19 and Communicable Disease Acknowledgment
I understand that attending a group event carries a risk of exposure to COVID-19 or other communicable diseases. I agree not to attend if I am experiencing symptoms or have tested positive within the last week.
By Reading this waiver? I certify that I have read and understood this waiver and release of liability. I understand that I am giving up certain legal rights, including the right to sue.
